
Staff Selection Commission (SSC) has updated the formats for Disability Certificates for Persons with Benchmark Disabilities (PwBD) following the latest guidelines from the Department of Empowerment of Persons with Disabilities (DEPwD) dated 16 October 2024. The new system now uses two streamlined forms: Form V for single disabilities and Form VI for multiple disabilities, replacing the earlier three-form structure.
These updated formats apply to all examinations notified after 16 October 2024, but candidates can still use older certificates during the transition period to avoid disqualification. The new forms also include UDID details, part of the government’s move to give every PwBD candidate a unique digital ID, ensuring a smoother and standardized certification process for upcoming SSC recruitments.
SSC has now reduced the previous three forms to just two:
Form V – For Single Disability: For candidates with only one disability.
Form VI – For Multiple Disabilities: For candidates with more than one disability.
These new forms are issued by a Medical Authority and follow the rules under Rule 18(1). They replace the older Forms V, VI, and VII used in past SSC notifications.

The SSC has said that the new disability certificate forms (Form V and Form VI) will be used for all exams where the recruitment notice was released after 16 October 2024.
To make the change easier, candidates can use either:
The new Form V or Form VI, or
The older Forms V, VI, or VII
This rule applies as long as the recruitment process is still going on.
SSC has made sure that candidates will not face any disadvantage because of the new forms:
People with older certificates can still use them.
New applicants can submit certificates in the updated forms.
Recruitment authorities will accept both old and new formats, so no one is rejected due to the certificate format.
Candidates are advised to check carefully which format applies to their exam before submitting the certificate.
Candidates must ensure their disability certificate is issued by a recognized medical authority and follows the latest SSC formats: Form V for single disabilities and Form VI for multiple disabilities. Originals must be carried to the exam for PwBD benefits; photocopies are not accepted. Certificates should include UDID details, and candidates must submit all required documents, like proof of residence and recent photos, before the deadline to avoid disqualification.
