• OTR Details: One-Time Registration details are required to apply.

• Photograph: A recent passport-size photograph in the required format.

• Signature: A clear scanned signature on a white background.

• Educational Certificates: High School, Intermediate, Graduation, and other required marksheets and certificates.

• B.Ed. Certificate: A B.Ed. degree from an NCTE-recognised institution is required. For Physical Education posts, B.P.Ed. or B.P.E. may be required.

• TET Certificate: Upper Primary Level TET (Classes 6 to 8) pass certificate with the required validity.

• Category Certificate: SC/ST, OBC, EWS, PwD, and other certificates, if applicable.

• Domicile Certificate: Required for candidates claiming reservation benefits in Uttar Pradesh.

• Other Certificates: Freedom Fighter Dependent Certificate and other certificates, if applicable.

• Payment Details: Credit card, debit card, or net banking details for fee payment.

• Reservation benefits are available only to permanent residents of Uttar Pradesh.

• Candidates from other states will be considered under the General category.

• Female candidates who want to claim caste reservation must submit a caste certificate issued from their father's side.

• The EWS certificate should be issued for the required year mentioned in the notification.

• Candidates should check all details carefully before submitting the form.

• Candidates should take 3 printed copies of the submitted application form.
