The National Testing Agency (NTA) has activated the RE-NEET Self Declaration Form 2026, and it is now available to download together with the Re-NEET UG 2026 admit card on the official portal, neet.nta.nic.in. Every candidate appearing in the NEET UG re-examination on Sunday, June 21, 2026 must download this undertaking through their candidate login and carry a filled, signed copy to the allotted exam centre.
The self-declaration form is not optional. It is one of the core documents checked during verification at the centre gate, and candidates who arrive without a correctly filled undertaking risk being held up — or turned away — during the document-check stage. With the re-test now less than 24 hours away, completing this form correctly is the single most important pre-exam task left for the roughly 22.79 lakh registered aspirants.
The RE-NEET Self Declaration Form 2026, also called the undertaking or proforma, is a candidate-level declaration printed as part of the admit card PDF. There is no standalone download link for it — it generates automatically when you download your Re-NEET UG 2026 admit card.
By filling and signing the form, a candidate formally confirms that they have read the NEET UG guidelines and instructions printed on the admit card and will abide by them. The page carries the candidate's personal particulars and includes dedicated boxes for a passport/postcard-size photograph, a left-hand thumb impression, and the candidate's signature. NTA advises every aspirant to read the printed instructions in full before reporting, so that nothing is left incomplete or wrongly filled on the morning of the exam.
This is a fresh document for the re-exam. The undertaking issued with the cancelled May 3 admit card is no longer valid — only the new June 21 admit card and its attached self-declaration form will be accepted.
Follow these steps to download the undertaking along with your hall ticket:
Go to the official website, neet.nta.nic.in.
Open the "RE-NEET UG 2026 Admit Card" download link.
Enter your Application Number and Password (or Date of Birth) with the security captcha.
Submit the login details.
Download the admit card PDF — the self-declaration / undertaking page is attached within it.
Take a clear printout (multiple copies are advisable).
Locate the undertaking page and keep it ready for filling.
If the portal is slow because of last-minute traffic, try logging in during off-peak hours and keep your application number and password handy in advance.
Filling the form is simple but must be done carefully — small errors here cause avoidable stress at the gate. Complete the following before exam day:
Read every exam-day instruction printed on the admit card and the undertaking.
Paste the same passport/postcard-size photograph you uploaded in your online application — it must match.
Place your left-hand thumb impression in the marked box.
Verify all printed personal details: name, roll number, application number, date of birth and exam centre address.
Do NOT sign in any field that must be completed in the presence of the invigilator.
The critical rule to remember: your signature on the attendance-linked section is made only at the centre, in front of the invigilator — not at home. Everything else (photo, thumb impression, detail verification) should be ready beforehand.
Along with the printed admit card and the filled self-declaration form, candidates must carry:
A valid original photo ID (Aadhaar, PAN, Passport, Voter ID or Driving Licence).
An additional passport-size photograph matching the application photo.
PwD/PwBD certificate and approved scribe documents, where applicable.
The Re-NEET UG 2026 examination will be held in offline, pen-and-paper mode on June 21, 2026, from 2:00 PM to 5:15 PM (195 minutes). Candidates should reach the centre well before the reporting time, follow the NTA dress code, and account for biometric verification and frisking, which are part of the tightened security protocol introduced after the May 3 paper-leak controversy that led to this re-test.
For any technical issue while downloading, candidates can contact the NTA helpline at 011-40759000 / 011-69227700. Students feeling anxious in these final hours can reach the MANAS mental-health helpline at 14416.
