History of Universal Health Coverage Day

The idea of universal health coverage was formally endorsed by the UN General Assembly in 2012 . Before that, the international community recognized health as a fundamental human right but had not focused on comprehensive systems that could provide continuous, affordable, and high-quality care to all. Later, in 2014 the Universal Health Coverage Coalition began celebrating December 12 as UHC Day to mark the UNGA's recognition of UHC's importance. Finally, in 2017 the UNGA officially designated December 12 as International Universal Health Coverage Day, solidifying its status as an annual global observance.

India's approach to universal health coverage has been ambitious yet challenging. With a population of over 142.86 crores , providing health services that reach everyone equitably requires a well-coordinated effort between central and state governments. Here are some challenges in achieving UHC in India:
  • High Out-of-Pocket Expenditure: In India, over 40% of healthcare spending is borne by individuals out-of-pocket, placing a significant financial burden on households.
  • Unequal Access to Healthcare Services: Disparities in healthcare access between urban and rural areas and between different states hinder the goal of equitable coverage.
  • Insufficient Infrastructure: Limited healthcare facilities and a shortage of medical professionals in rural and remote areas impact service delivery.
  • Quality of Care: While access has improved, the quality of healthcare services remains inconsistent across regions, affecting patient outcomes.
  • Awareness and Education : A lack of awareness about health rights and services among the population can result in underutilization of available healthcare resources.
Despite these challenges, India has launched several schemes and policies to promote UHC.

Universal Health Coverage Schemes in India

India’s journey toward UHC is supported by various government schemes and policies aimed at ensuring affordable and quality healthcare for all. These include:
  • Ayushman Bharat: Launched in 2018, is the flagship scheme aimed at providing health coverage to economically vulnerable sections of society. Under this program, eligible families can access up to ₹5 lakh annually for secondary and tertiary care hospitalization.
  • National Health Mission : Launched in 2013, focuses on providing universal access to healthcare, especially in rural and underserved areas. The mission supports the development of health infrastructure and human resources to strengthen the healthcare delivery system.
  • Pradhan Mantri Bhartiya Janaushadhi Pariyojana (PMBJP) : It aims to provide quality medicines at affordable prices through a network of Jan Aushadhi Kendras. It helps in reducing out-of-pocket expenditure on healthcare for millions of Indian
  • Employees’ State Insurance: Offers social security and health insurance benefits to workers in the organized sector by providing comprehensive medical care, and cash benefits during sickness, maternity, and employment injuries.
