Anatomy of the Peritoneum

1. Parietal Peritoneum : This outer layer lines the walls of the abdomen and pelvis, creating a boundary for the abdominal cavity.
2. Visceral Peritoneum : This inner layer snugly covers the surfaces of organs like the stomach, intestines, and liver, keeping them protected.

Special Features of the Peritoneum

• Mesentery : Imagine a sheet that attaches the intestines to the back of the abdominal wall. That’s the mesentery, which helps hold the intestines in place while also supplying them with blood and nerves.
• Omentum : These are fatty layers of the peritoneum, often described as a protective apron. The greater and lesser omentum not only store fat but also play a role in fighting infections.
• Peritoneal Ligaments : Think of these as connective bands that help anchor organs in place or connect them to each other.

What Does the Peritoneum Do?

1. Support : It keeps the abdominal organs organized and secure.
2. Protection : The fluid within the peritoneal cavity cushions the organs, shielding them from mechanical injury.
3. Immunity : The peritoneum helps the body fight infections by isolating and containing them. The omentum even moves to sites of infection or injury like a "first responder."
4. Regulation : It manages the movement of fluids and electrolytes, ensuring a balance within the abdominal cavity.

Types of Organs Based on Peritoneal Coverage

• Intraperitoneal Organs : Fully wrapped by the visceral peritoneum, like the stomach and spleen.
• Retroperitoneal Organs : Partially covered and positioned behind the peritoneum, such as the kidneys and pancreas.
• Secondarily Retroperitoneal Organs : These start off fully surrounded by the peritoneum during development but later shift behind it, like parts of the colon.

Conditions Affecting the Peritoneum

1. Peritonitis : This is an infection or inflammation of the peritoneum, often caused by a ruptured appendix or other abdominal injury. Symptoms include severe pain, fever, and tenderness.
2. Ascites : A buildup of fluid in the peritoneal cavity, commonly seen in conditions like liver disease or cancer. It can cause swelling and discomfort.
3. Peritoneal Carcinomatosis : When cancer spreads to the peritoneum, it can result in advanced and challenging cases to treat.
4. Peritoneal Dialysis : For people with kidney failure, the peritoneum can act as a natural filter during dialysis, helping to remove waste from the body.
