Steps to Fill NEET 2025 Self Declaration Form

Filling the NEET Self Declaration 2025 Form is an easy process. Follow these steps carefully:

• Visit the Official Website: Go to neet.nta.nic.in.

• Log In: Use your application number and date of birth to log in.

• Download Admit Card: Click on the link to download the NEET 2025 admit card along with the Self Declaration Form.

• Fill the Form Manually: Take a printout and fill in the required details carefully.

• Fill the Personal Detailes: Fill in your name and your address where are you belong.

• Paste Photograph: Attach a passport-size photograph in the designated space. This should be the same as the one uploaded during the application process.

• Add Thumb Impression: Carefully place your left-hand thumb impression in the given box.

• Sign in Front of Invigilator: There is a signature box at the bottom of the form, but this must be signed only in front of the exam invigilator at the center.

NTA has provided important instructions for candidates to follow on the day of the NEET 2025 exam, along with the admit card. Here are the instructions candidates must follow on the exam day:

• Documents to Carry: Candidates need to bring the NEET 2025 admit card, undertaking, a valid ID, passport-sized, and postcard-sized photographs.

• Reporting Time: Candidates should arrive at the NEET 2025 exam center at the time mentioned on the admit card. No candidate will be allowed to enter the exam center after the gates close.

• Allowed Items: Besides the necessary documents, candidates can bring transparent water bottles, hand sanitizer bottles, extra photographs, PwD certificates, and scribe-related documents if needed, inside the NEET exam center.

• Prohibited Items: Candidates must not bring any banned items to the exam center on the NEET 2025 exam day. Prohibited items include stationery, electronic devices, food, accessories, jewellery, and bags.
