Filling the NEET Self Declaration 2025 Form is an easy process. Follow these steps carefully:

  • Visit the Official Website: Go to neet.nta.nic.in.

  • Log In: Use your application number and date of birth to log in.

  • Download Admit Card: Click on the link to download the NEET 2025 admit card along with the Self Declaration Form.

  • Fill the Form Manually: Take a printout and fill in the required details carefully.

  • Fill the Personal Detailes: Fill in your name and your address where are you belong.
  • Paste Photograph: Attach a passport-size photograph in the designated space. This should be the same as the one uploaded during the application process.

  • Add Thumb Impression: Carefully place your left-hand thumb impression in the given box.

  • Sign in Front of Invigilator: There is a signature box at the bottom of the form, but this must be signed only in front of the exam invigilator at the center.

  • Allowed Items: Besides the necessary documents, candidates can bring transparent water bottles, hand sanitizer bottles, extra photographs, PwD certificates, and scribe-related documents if needed, inside the NEET exam center.

  • Prohibited Items: Candidates must not bring any banned items to the exam center on the NEET 2025 exam day. Prohibited items include stationery, electronic devices, food, accessories, jewellery, and bags.
